What is a Home Care Package?

A Home Care Package is an Australian government funded support at home program that provides funding to seniors for the services necessary to live independently at home.

A Commonwealth government Home Care Package (HCP) will subsidise the costs of in-home aged care services.

Services included in a Home Care Package can range from personal care and domestic assistance to nursing services and social support. These packages are flexible, and you choose the services that suit your specific needs.

How do I Get a Home Care Package?

To get a government Home Care Package you need to be assessed by the Aged Care Assessment Team (ACAT). To be assessed you need to apply at MyAgedCare.gov.au/help-at-home. An ACAT assessor will visit your home to discuss your needs and eligibility.

Once you have been assessed as eligible to receive a Commonwealth Home Care Package, you will go on to a waiting list until a package becomes available for you. You will also receive an approval letter stating your HCP level of funding 1,2 3, or 4.

While you are waiting for your package you can research Commonwealth Home Care Package providers, like Mayflower, to see what services we can offer.

Self-Managed Home Care Packages

One of the unique features of Mayflower Home Care is our Self-Managed Home Care option.

When you self-manage your HCP, you are choosing minimal case management support. Mayflower will still provide you with a case manager to do the initial set up of your services and be available for further support.

You will be provided with flexible options to choose your own service providers and the way your services are delivered. Mayflower will also do safety and assurance checks, such as police and insurance checks for all contracted services recommended to you.

In addition, we will provide annual home visits to make sure your Home Care Package is providing the right care for you.

The Advantages of a Self-Managed Home Care Package Include:

Lower fees- Case management costs are reduced to a flat 12% rate, rather than charging an hourly case management fee.

Freedom of choice- You have more choice of what your Home Care Package is used for, ensuring that you receive the care you need on your terms. You will have greater control over your care services, choosing how and when they are delivered.

Case Managed Home Care Packages

Mayflower clients can choose a fully case managed HCP. This option provides you with a case manager to support your Home Care needs and services.

Managed Home Care requires a fee for case management hours of between 18% to 20% funded through your HCP. With managed Home Care, there are more case manager home visits and support for your carers.

Mayflower will also coordinate the services provided to you.

Better Ratios for Individualised Care

Our case managers have qualifications in nursing and community care, so they can support you whether you choose a self-managed or fully managed Home Care package.

The ratios are 45 clients per full-time case manager, and ratios reduce determined by the days your case manager works. Other providers have a ratio of 80 clients per case manager regardless of the hours they work.

Mayflower’s favorable case manager ratios ensure you receive personalised, flexible support, making it easy to access your case manager whenever you need them.
